Core clinical principle

CBT-I is compatible with and effective for chronic disease + insomnia. The old model — "treat the disease first, then the sleep" — has been overturned. Treating insomnia independently improves both sleep and disease outcomes. Do not delay.

India — the scale of the problem

India carries a disproportionate burden of all five disease categories in this pathway: 77M diabetics, 220M hypertensives, ~56M with depression, and the world's largest absolute burden of musculoskeletal conditions. Sleep treatment at scale for these populations represents one of the highest-leverage public health interventions available.
